What Is REACH?

REACH Academy & Resource Center is Luzerne County’s referral navigation hub. We help residents figure out where they should go, complete an intake, and then make a warm handoff to partner agencies like yours.

REACH does intake, needs review, documentation help, and follow-up so your staff can stay focused on service delivery. We are not a treatment provider or direct-service agency — we are your front-end support team.

How We Work With Partner Agencies

1. Streamlined Intakes

Residents arrive at your agency with:

  • Completed consent and basic information summarized
  • A clear understanding of what your program provides
  • Any required documents we were able to help them gather

2. Accurate Referral Matching

Every resident is matched using:

  • Your eligibility rules and program restrictions
  • Capacity updates you share with us
  • The correct pathway category (recovery, housing, workforce, education, family support)

3. Warm Handoffs

We contact your agency directly through your preferred method:

  • Phone introduction
  • Email handoff
  • Online referral form
  • Scheduling an intake appointment

4. Follow-Up Support

REACH checks in with residents within 24–72 hours (depending on urgency), confirms they connected, and helps them complete any next steps you identify.

5. Less Load on Your Staff

REACH reduces:

  • Misdirected walk-ins and repeated phone calls
  • Residents arriving without basic documents or information
  • No-shows caused by confusion about where to go or what to bring

What We Ask From Partners

1. Share Your Intake Rules

Please provide:

  • Eligibility requirements
  • Required documentation
  • Intake hours and locations
  • Preferred referral methods
  • Program restrictions (age, gender, county, etc.)

2. Provide Capacity Updates

When possible, let us know about waitlists, seasonal changes, or temporary closures so we do not send residents to unavailable programs.

3. Accept Warm Handoffs

Work with us to receive residents through:

  • Phone introductions
  • Email referrals
  • Your own online forms
  • Scheduled appointments

4. Share Minimal Outcome Notes

We do not request clinical notes or case files. We only ask, when appropriate, for a simple outcome such as:

  • Connected
  • Did not connect / no-show
  • Needs additional documentation
  • Reschedule required

What REACH Does Not Do

To protect your role and keep responsibilities clear, REACH does not:

  • Provide treatment or counseling
  • Offer housing units or direct case management
  • Deliver medical, legal, or emergency services
  • Request or store medical records or clinical notes
  • Replace or duplicate any existing agency services

REACH is a navigator, not a service provider. Our role is to make your programs run smoother — never heavier.

The REACH Referral Workflow

Step 1 — Resident Intake

Resident completes intake with REACH by walk-in, phone, or online. Consent is obtained and needs are documented.

Step 2 — Pathway Assignment

We determine the primary pathway:

  • Recovery & Behavioral Health
  • Housing & Stability
  • Employment & Workforce
  • Education & Credentials
  • Family Support & Essentials

Step 3 — Partner Matching

Navigator selects the best-fit partner based on eligibility, capacity, geography, and program fit, using the partner directory you help us maintain.

Step 4 — Warm Handoff

REACH completes a warm handoff to your agency by phone, email, online form, or appointment scheduling and gives the resident clear instructions for next steps.

Step 5 — Follow-Up

REACH follows up with the resident within 24–72 hours (depending on urgency) to confirm connection and support any additional steps you identify.
