- Alignment: REACH checks eligibility, geography, urgency, and partner criteria.
- Warm Handoff: Partner receives only what’s required to begin intake.
- Follow-Up: REACH confirms first contact and helps with next steps if needed.
- Feedback Loop: Resident can share a brief experience note to improve routing.
Privacy: REACH shares minimum necessary information for service connection. Deletion requests are honored when legally permissible.
