REACH is Luzerne County’s referral navigation hub. We prepare residents, confirm basic eligibility, and complete warm handoffs so your team receives the right people with clear intake expectations.

What Is REACH?

The REACH Resource Center is a countywide “front door” for guidance and referrals. Luzerne County residents come to REACH first to describe their needs, complete intake and consent, and understand which partner program fits them.

We are not a treatment provider or direct-service agency. Our role is referral navigation, documentation help, and follow-through so partner agencies can focus on delivering services.

How We Work With Partner Agencies

1) Streamlined Intakes

When residents arrive at your agency, they come with:

  • Basic information and consent already completed
  • A clear understanding of your program and what to expect
  • Required documents we were able to help gather

2) Accurate Referral Matching

Requests are matched using:

  • Your eligibility rules and program restrictions
  • Capacity updates you share with us
  • The correct pathway category (recovery, housing, workforce, education, family support)

3) Warm Handoffs

We contact your agency directly using your preferred method:

  • Phone introductions
  • Email referrals
  • Your online referral form
  • Scheduling an intake appointment

4) Follow-Up Support

REACH checks in with residents within 24–72 hours (based on urgency) to confirm they connected, and help complete any next steps you identify.

5) Less Load on Your Staff

REACH reduces:

  • Misdirected walk-ins and repeated phone calls
  • Residents arriving without key documents or information
  • No-shows caused by confusion about where to go or what to bring

What We Ask From Partners

1) Share Your Intake Rules

Please provide:

  • Eligibility requirements
  • Required documentation
  • Intake hours and locations
  • Preferred referral methods
  • Program restrictions (age, gender, county, etc.)

2) Provide Capacity Updates

When possible, share waitlists, seasonal changes, or temporary closures so we do not send residents to unavailable programs.

3) Accept Warm Handoffs

Work with us to receive residents through:

  • Phone introductions
  • Email referrals
  • Your own online forms
  • Scheduled appointments

4) Share Minimal Outcome Notes

We do not ask for clinical notes. We only request simple outcomes such as:

  • Connected
  • Did not connect / no-show
  • Needs additional documentation
  • Reschedule required

What REACH Does Not Do

To keep responsibilities clear, REACH does not:

  • Provide treatment or counseling
  • Offer housing units or direct case management
  • Deliver medical, legal, or emergency services
  • Request or store medical records or clinical notes
  • Replace or duplicate existing agency programs

REACH is a navigator, not a service provider. Our job is to make your programs run smoother — never heavier.

The REACH Referral Workflow

Step 1 — Resident Intake

Resident completes intake with REACH by walk-in, phone, or online. Consent is obtained and needs are documented.

Step 2 — Pathway Assignment

We determine the primary pathway:

  • Recovery & Behavioral Health
  • Housing & Stability
  • Employment & Workforce
  • Education & Credentials
  • Family Support & Essentials

Step 3 — Partner Matching

Navigator selects the best-fit partner based on eligibility, capacity, geography, and program fit using the directory you help us maintain.

Step 4 — Warm Handoff

REACH completes a warm handoff by phone, email, online form, or appointment scheduling and gives the resident clear next steps.

Step 5 — Follow-Up

REACH follows up within 24–72 hours to confirm connection and support any additional steps you identify.
