Purpose
The MOU establishes shared goals and coordination standards between REACH and each participating partner. It ensures referrals are accurate, ethical, and transparent.

Who Uses This
This framework applies to any organization receiving REACH referrals or participating in the coordination system, including public, private, nonprofit, and faith-based providers.

1) Responsibilities of REACH
- Coordinate, document, and route referral requests
- Share only minimum necessary information for intake
- Maintain up-to-date eligibility, hours, and intake methods
- Provide simple follow-up notes to confirm connection
2) Responsibilities of the Partner
- Accept referrals within agreed categories
- Keep service details and eligibility rules current
- Report basic outcomes (connected / no-show / needs docs)
- Protect confidentiality and follow applicable laws
3) Mutual Terms
- Neither party assumes liability for the other’s services
- No financial obligation unless stated in writing
- Concerns are addressed through dialogue before termination
4) Duration & Renewal
- Effective for one year from signature date
- Renewable upon mutual review of collaboration
