Compassion

Treat each person with respect and empathy, recognizing that anyone can need support at any stage of life.

Collaboration

Strengthen existing programs by building coordinated referral pathways that reduce confusion and delays.

Access & Inclusion Statement

Access & Inclusion: REACH values diversity and works to ensure equitable access for all residents. Materials are built for accessibility and mobile use. Spanish-language options are planned as categories go live.

Why a Referral Model

Luzerne County has strong programs. The barrier is navigation. People often face a maze of agencies, eligibility rules, and shifting waitlists. REACH serves as a front door and guide, matching residents to partners using clear criteria and a simple intake.

REACH is a navigator, not a direct-service provider. Partners deliver the supports residents need.

Referral Workflow (Design)

  1. Intake: Residents share needs and contact preferences online, by phone, or in person.
  2. Alignment: Requests are matched by category, eligibility, geography, and urgency.
  3. Warm Handoff: The right partner receives only what’s needed to begin service.
  4. Follow-Through: REACH confirms first contact and supports next steps if needed.
