REACH is a referral navigation hub. We do not provide direct treatment or program services. We help residents identify the right category, complete a simple intake, and receive a warm handoff to a trusted partner.
Based at The REACH Resource Center in Kingston, PA. Partners appear in the directory once MOUs are signed.

What We Share

Pathways We Coordinate
Recovery & Treatment
Connections to detox, inpatient, outpatient, MAT, peer support, and recovery housing partners.
Housing & Stability
Navigation for shelters, rapid rehousing, eviction prevention, utilities, and rental support providers.
Education & Training
GED prep, adult education, tutoring, ESL, credentials, and skill-building programs.
Employment & Reentry
Job readiness, CareerLink-type services, second-chance hiring, apprenticeships, and workforce training.
Mental Health & Wellness
Counseling, therapy, crisis resources, and ongoing wellness supports through licensed or community partners.
Family & Community Support
Childcare supports, parenting resources, food/clothing assistance, transportation help, and safety services.
