Connecting Luzerne County to the Help It Needs

REACH — Real Educational Answers for Community Hope operates through The REACH Resource Center in Kingston, Pennsylvania. Our focus is simple: make it easier for Luzerne County residents to find trustworthy local help through a clear, coordinated referral network.

REACH aligns community partners across recovery, housing, education, employment, and family support so residents can identify the right next step without navigating a maze of agencies.

REACH is a navigator, not a service provider. We do intake, alignment, and warm handoffs to partner agencies; we do not provide treatment, housing units, direct case management, or emergency services.

How the REACH Referral Workflow Works

1) Request
You, or someone on your behalf, tells us what you need and how to contact you.

2) Alignment
REACH reviews your situation and assigns a primary pathway: recovery, housing, employment, education, or family support.

3) Handoff
We complete a warm handoff to a trusted partner agency using their preferred referral method.

4) Follow-Up
REACH checks back within 24–72 hours, depending on urgency, to confirm connection and next steps.

5) Feedback
Simple outcomes (connected, no-show, needs documents, reschedule) help improve future referrals.

Why This Matters

Strong programs already exist in Luzerne County. Finding the right door is the challenge. Coordination reduces confusion, shortens time to help, and strengthens collaboration among providers.

By staying strictly in a navigation role, REACH supports agencies instead of duplicating their services.
